Given the number of inquiries and donation requests, we have set up this questionnaire to learn more about your organization and the donation request.  

Once your form is received and reviewed, a member of our team will be in touch with a decision for your request. 

We would love to be able to say yes to all; however, due to the number of requests and our allotted budget for donations, not all requests will receive a yes each year. 

If a donation is honored by Creekhaven Inn & Spa, only one donation per year will be allowed.
